Milton Mall Survey 2023 Sweepstake

No Purchase Necessary to Enter or Win. 

ELIGIBILITY: INDIVIDUALS, 18 OR OLDER, WHO ARE LEGAL RESIDENTS OF CANADA . VOID WHERE PROHIBITED. Employees (and their immediate families (parent, child, spouse or sibling and their respective spouses, regardless of where they reside) and those living in their same households, whether or not related) of simplydbs inc. (“simplydbs”) and their parent, subsidiaries, divisions, affiliated companies, dealers and advertising and promotion agencies, are not eligible to enter or win. By participating, entrants agree to be bound by these Official Rules and the decisions of the simplydbs and judges, which are binding and final on matters relating to this sweepstake. Sweepstake is subject to all applicable federal, provincial and local laws.

SWEEPSTAKES PERIOD: The sweepstake begins at 9AM EST Eastern Time (“ET”) on January 13 and ends at 11:59 PM ET on February 2, 2024 (“Sweepstake Period”). 

TO ENTER: During the Sweepstake Period, there is one (1) way to enter. Survey Entry: Click on the link provided. Limit one (1) entry per person/email address/household for the entire Sweepstake Period. Multiple entries will be void. Entries generated by script, mechanical, macro or other automated means or by any means which subvert the entry process are void.

DRAWING: Potential winner(s) will be selected in random drawing by February 23, 2024 from all eligible entries received for the applicable drawing. There will be two (2) random drawing: 1, $250 CAD Funvilla (Milton Mall location) and 1, $500 CAD a vendor of choice at Milton Mall. There will be 2 winners in total. Odds of winning depends on the number of eligible entries received for the applicable drawing. Potential winner will be notified by email and will be required to execute and return a Release Form within 2 weeks of being notified of as a winner. Winner will also be required to answer a time-limited, mathematical skill-testing question. Failure to answer the question correctly within the time period specified, as determined by simplydbs in its sole discretion, will result in disqualification and, at simplydbs sole discretion, the prize may be awarded to an alternate winner for the drawing at issue. If a prize or prize notification is returned as undeliverable, or a potential winner does not respond to the initial notification within the time period stated or a potential winner is not in compliance with these rules, prize will be forfeited and, at simplydbs’ discretion and time permitting, an alternate winner selected for the drawing at issue.

PRIZES

(A)   1 x $250 CAD gift card to Funvilla at Milton Mall; 1 x $500 CAD to vendor of choice located at the Milton Mall. Two potential winners will be selected. 

Prizes are awarded “as is'' with no warranty or guarantee, either express or implied. All federal, provincial, state and local taxes (including any income and withholding taxes), if any, and all other costs and expenses associated with prize acceptance and use not specified herein as being provided are the sole responsibility of the winners. Winners may not substitute, assign or transfer a prize, but simplydbs reserves the right, at its sole discretion, to substitute a prize of comparable or greater value. All prize details are at simplydbs sole discretion. Limit one (1) prize per person/email.

PUBLICITY GRANT AND PERSONAL INFORMATION: Each winner, by acceptance of prize, except where legally prohibited, grants permission for simplydbs and its designees to use his/her name, address (city and state), photograph, voice and/or other likeness and prize information for advertising, trade and promotional purposes without further compensation, in all media now known or hereafter discovered, in any manner, worldwide in perpetuity, without notice or review or approval. In addition, by entering the Sweepstake or accepting the Prize, each entrant consents to the collection, use and disclosure by simplydbs, its agents or representatives of such entrant’s personal information for the following purposes: (a) administering (including, without limitation, in submitting any filing required in connection with the Sweepstake with a regulatory body) and publicizing the Sweepstake; (b) detecting and protecting simplydbs, its affiliates, and other third parties against error or fraud and other illegal activity; (c) as otherwise disclosed to the entrant and for which simplydbs obtains the entrant’s consent; and (d) as permitted or required by law, all without payment or consideration. The Milton Mall Survey is conducted by simplydbs inc. (“simplydbs”) and is subject to simplydbs’ Privacy Policy. The survey is powered by SurveyMonkey service. Your survey answers, including personal information, will be stored with SurveyMonkey for as long as reasonably necessary to gather and compile the results of the survey. LIMITATIONS OF LIABILITY: Released Parties (as defined below) are not responsible for lost, late, incomplete, inaccurate, stolen, delayed, misdirected, undelivered, or garbled messages, notifications, mail or entries; or for lost, interrupted or unavailable network, server, Internet Service Provider (ISP), website, or other connections, availability or accessibility or miscommunications or failed computer, satellite, telephone or cable transmissions, lines, or technical failure or jumbled, scrambled, delayed, or misdirected transmissions or computer hardware or software malfunctions, failures or difficulties, or other errors or difficulties of any kind whether human, mechanical, electronic, computer, network, typographical, printing or otherwise relating to or in connection with the sweepstakes, including, without limitation, errors or difficulties which may occur in connection with the administration of the sweepstake, the processing of entries, the announcement of the prizes or in any sweepstake-related materials. Released Parties are also not responsible for any incorrect or inaccurate information, whether caused by site users, tampering, hacking, or by any equipment or programming associated with or utilized in the sweepstake. Released Parties are not responsible for injury or damage to participants' or to any other person's computer related to or resulting from participating in this sweepstake or downloading materials from or use of the website. Persons who tamper with or abuse any aspect of the sweepstake or website or who are in violation of these Official Rules, as solely determined by simplydbs, will be disqualified and all associated entries will be void. simplydbs reserves the right at its sole discretion to suspend, modify or terminate the sweepstakes should any portion of the sweepstakes be, in simplydbs’ sole opinion, compromised by virus, worms, bugs, non-authorized human intervention or other causes which, in the sole opinion of the simplydbs, corrupt or impair the administration, security, fairness or proper play, or submission of entries. If terminated, simplydbs may, at its discretion, select the potential winners from all eligible, non-suspect entries received prior to action taken or as otherwise deemed fair and appropriate by simplydbs. Entrants, by participating, agree that simplydbs, their parent, subsidiaries, divisions, affiliated companies, service agencies, independent contractors, advertising and promotion agencies, web hosting and service and fulfillment providers, property managers and clients, and all of their respective officers, directors, employees, representatives and agents (collectively, “Released Parties”) will have no liability whatsoever for, and shall be held harmless by entrants against, any liability, for any injuries, losses or damages of any kind, including death, to persons, or property resulting in whole or in part, directly or indirectly, from acceptance, possession, misuse or use of a prize or participation in this sweepstakes. In the event of a dispute regarding entries received from multiple users having the same email account, the authorized subscriber of the email account at the time of entry will be deemed to be the entrant and must comply with these rules. Authorized account subscriber is the natural person who is assigned the email address by the Internet Service Provider (ISP), on-line service provider, or other organization responsible for assigning email addresses. For individuals in the province of Quebec: Any litigation respecting the conduct or organization of a publicity contest may be submitted to the Régie des alcools, des courses et des jeux for a ruling. Any litigation respecting the awarding of a prize may be submitted to the board only for the purpose of helping the parties reach a settlement.
